Personal trainer contract template
Trainers sell time in advance and lose it to late cancellations. This agreement sets the package, the cancellation window and the health disclosures in one page a client can sign on their phone.
The template
1. PackageClient purchases [10] sessions of [60] minutes for [amount], paid in advance. Sessions expire [90] days from purchase. Unused sessions are not refundable but may be transferred once to another person.
2. Scheduling and cancellationSessions are booked by [app / text]. Cancellations with less than [24] hours' notice, and no-shows, are charged as a full session. Trainer gives the same courtesy: a session Trainer cancels late is replaced free.
3. Late arrivalSessions end at the scheduled time regardless of start time. Arrivals more than [15] minutes late may be treated as a cancellation.
4. Health disclosureClient has completed the health questionnaire, will tell Trainer about any change in health, injury or medication, and confirms they are cleared to exercise or will obtain clearance from a doctor.
5. Assumption of riskExercise carries a risk of injury. Client takes part voluntarily, will follow instructions, and will stop and tell Trainer if something hurts. Trainer holds [liability insurance] and Trainer's liability is limited to the fees paid.
6. Payment and renewalPackages are paid before the first session. Renewal at the same rate is offered before the final session. Rates are reviewed every [12] months.

Prepay, always
Selling sessions one at a time means chasing money every week. A prepaid package with an expiry date is fair to the client, funds your month, and makes the cancellation rule enforceable because the session is already paid for.
Twenty-four hours, both ways
Clients accept a strict cancellation rule when it cuts both ways. Offering a free replacement for anything you cancel late costs almost nothing and removes every objection.
This is not a waiver
Clause 5 records that the client understands the risk; it does not replace a proper liability waiver or your insurance. Use your insurer's waiver alongside it if they provide one.
